Glaucoma surgery can cause slight macular thickening due to reduced intraocular pressure. This optical coherence tomography (OCT) study found no significant differences based on medications or mitomycin C use.
Area of Science:
- Ophthalmology
- Glaucoma Surgery
- Macular Edema
Background:
- Glaucoma filtration surgery aims to reduce intraocular pressure.
- Postoperative macular changes after surgery require monitoring.
Purpose of the Study:
- To analyze early postoperative macular changes using optical coherence tomography (OCT) after uncomplicated glaucoma filtration surgery.
- To investigate the impact of medications and mitomycin C on these changes.
Main Methods:
- Prospective study of 32 patients (34 eyes) with open-angle glaucoma undergoing trabeculectomy.
- Optical coherence tomography (OCT) used for macular thickness analysis at baseline, 2nd day, 1 week, and 1 month post-surgery.
Main Results:
- A slight increase in macular thickness was observed 1 week and 1 month post-surgery compared to baseline and the 2nd day.
- No significant difference in macular thickness changes was found between patients using topical prostaglandins versus other topical medications.
- No significant difference in macular changes was observed between patients treated with or without mitomycin C.
Conclusions:
- Uncomplicated trabeculectomy may lead to a slight, subclinical increase in macular thickness.
- This thickening is likely a physiological response to the sudden reduction in intraocular pressure.
- Macular thickening is a consequence of altered pressure dynamics within the retinal capillaries.
