Summary

Heterotopic ossification (HO) involves abnormal bone growth in soft tissues, impacting patient recovery. This study identifies Bone Morphogenetic Protein-9 (BMP-9) as a key factor in traumatic HO development.

Area of Science:

  • Orthopedics
  • Regenerative Medicine
  • Biochemistry

Background:

  • Heterotopic ossification (HO) is the abnormal formation of mature bone in soft tissues, particularly skeletal muscle.
  • HO in polytraumatized patients leads to significant morbidity, negatively affecting functional outcomes and rehabilitation.
  • HO necessitates surgical interventions, increasing healthcare costs.

Purpose of the Study:

  • To investigate the molecular mechanisms underlying traumatic heterotopic ossification.
  • To identify specific bone morphogenetic proteins (BMPs) involved in HO formation.
  • To report the first case of BMP-9 expression in traumatic HO.

Main Methods:

  • Case presentation of a 34-year-old male with severe hip HO post-trauma.
  • Immunohistochemical analysis of resected HO tissue.
  • Detection and quantification of BMP expression within the ossified tissue.

Main Results:

  • Severe heterotopic ossification was observed around the right hip 11 days after major trauma.
  • Immunohistochemical analysis revealed the expression of several BMPs in the HO tissue.
  • Highly osteogenic BMP-9 was notably expressed in the affected soft tissue.

Conclusions:

  • This is the first report demonstrating local BMP expression, specifically BMP-9, in traumatic HO.
  • BMP-9 may contribute to soft tissue HO formation, potentially via mesenchymal stem cells (MSCs).
  • The presence of a suitable microenvironment is crucial for BMP-9-mediated HO in soft tissues.
