    Chlorhexidine gluconate (CHG) can cause severe skin burns in extremely low birth weight (ELBW) infants. Wiping the antiseptic solution off the skin may help prevent these adverse reactions.

    • Chlorhexidine gluconate (CHG) is a broad-spectrum antiseptic frequently used in neonatal intensive care units (NICUs).
    • Limited safety data exists for CHG use in neonates, particularly preterm infants where skin integrity is crucial for preventing sepsis and other complications.

    Observation:

    • This report details two cases of severe skin burns in extremely low birth weight (ELBW) infants following umbilical cord sterilization with CHG before umbilical line placement.
    • The affected infants weighed 610 g and 600 g at birth.

    Findings:

    • A significant association exists between CHG use and skin burns in the ELBW population.
    • Promptly wiping the CHG solution from the infant's skin appears to mitigate the risk of burn injury.
