This study reports a rare case of giant retroperitoneal liposarcoma with leiomyosarcomatous differentiation in a 60-year-old man. The patient experienced multiple recurrences despite surgical resection and radiotherapy.

  • Retroperitoneal liposarcomas are rare malignant tumors.
  • Giant liposarcomas pose surgical challenges due to their size and location.

Observation:

  • A 60-year-old Japanese man presented with dysphagia and abdominal distention due to a giant retroperitoneal mass.
  • Computed tomography revealed a large, predominantly lipomatous tumor with heterogeneous solid components.
  • Surgical resection was performed, and histopathology showed well-differentiated and dedifferentiated liposarcoma with leiomyosarcomatous differentiation.

Findings:

  • The tumor exhibited a rare heterologous leiomyosarcomatous component within the liposarcoma.
  • The patient survived for 3 years post-treatment but experienced three local recurrences.
