Rare postpartum ruptured degenerated fibroid: a case report

Background:

• Spontaneous uterine fibroid rupture is an uncommon obstetric and gynecologic emergency.
• This condition is particularly rare in the postpartum period.

Observation:

• A 31-year-old woman presented with acute abdominal pain 9 weeks postpartum.
• Physical examination revealed diffuse abdominal tenderness and rebound tenderness.
• Ultrasonography identified a large uterine fibroid and significant free fluid in the abdomen.

Findings:

• Exploratory laparotomy revealed a ruptured, cystic degenerated uterine fibroid with active hemorrhage.
• Approximately 500 mL of blood-stained peritoneal fluid and pus were present.
• Myomectomy was performed, with successful evacuation of fluid and clots.

Implications:

• Preoperative diagnosis of spontaneous uterine fibroid rupture with intra-abdominal hemorrhage is challenging.
• Exploratory laparotomy serves as a crucial diagnostic and therapeutic intervention.
• Prompt surgical management is vital for this rare, life-threatening condition.
