Area of Science:

  • Endocrine Surgery
  • Minimally Invasive Surgery
  • Surgical Oncology

Background:

  • Gasless transaxillary endoscopic thyroidectomy (ET) is a novel surgical approach.
  • Over 1000 operations have been performed using this technique since its development in 2001.

Purpose of the Study:

  • To document surgical outcomes after a decade of experience with gasless transaxillary ET.
  • To evaluate the current role of endoscopic thyroidectomy in surgical practice.

Main Methods:

  • A retrospective analysis of patients undergoing elective gasless transaxillary ET from November 2001 to December 2010.
  • Evaluation of patient demographics, surgical data, histologic findings, and complications.

Main Results:

  • 1085 patients underwent ET, with a majority (857) having thyroid malignancies.
  • The overall complication rate was 13.5%, including transient hypocalcemia (6.3%) and hoarseness (3.9%).
  • Permanent complications were rare, with permanent hypocalcemia at 0.1% and recurrent laryngeal nerve injury at 0.2%.
  • Conclusions:

    • Gasless ET demonstrates surgical outcomes and complication rates comparable to open thyroidectomy.
    • Endoscopic thyroidectomy serves as a crucial intermediate between conventional open surgery and robotic techniques.
