Summary

This study shows that setting specific targets for cochlear implant (CI) programming is feasible. The Fitting to Outcomes Expert (FOX) system effectively improves CI fitting success rates across multiple centers.

Area of Science:

  • Audiology
  • Biomedical Engineering
  • Otorhinolaryngology

Background:

  • Cochlear implant (CI) fitting requires optimizing multiple psychoacoustic outcomes.
  • Standardized methodologies for achieving these outcomes across diverse patient groups and clinical settings are needed.

Purpose of the Study:

  • To demonstrate the feasibility of defining psychoacoustic outcome targets for CI users.
  • To evaluate a systematic methodology for achieving these targets in a multicenter study.
  • To assess the effectiveness of the Fitting to Outcomes Expert (FOX) system in CI programming.

Main Methods:

  • Retrospective analysis of data from 255 adult and pediatric CI users across 14 centers.
  • Utilized the Advanced Bionics HiRes90k cochlear implant.
  • Employed a target-driven fitting approach using the FOX system.
  • Measured 66 psychoacoustic outcomes, reduced to 22 summary variables, comparing initial and latest results.

Main Results:

  • The FOX system significantly improved the proportion of psychoacoustic variables meeting preset targets.
  • Over half (57%) of targets were met at initial device activation with automated MAPs.
  • Application of the FOX system resulted in a significant 24% increase in achieved targets (P < 0.001).

Conclusions:

  • Defining and reporting on CI fitting effectiveness using outcome targets is feasible.
  • The FOX system offers an effective tool for systematic CI programming and optimization.
  • A standardized target-driven approach facilitates consistent, high-quality CI programming across multiple clinical centers.
