Summary

Encainide and flecainide are class IC antiarrhythmic drugs that effectively suppress ventricular arrhythmias. However, patient response to one drug does not predict response to the other, despite similar electrophysiologic profiles.

Background:

  • Class IC antiarrhythmic agents, encainide and flecainide, are sodium channel blockers.
  • These agents are effective in suppressing ventricular arrhythmias by slowing cardiac conduction.

Purpose of the Study:

  • To compare the antiarrhythmic effects of encainide and flecainide.
  • To determine if response to one agent predicts response to the other in patients with malignant ventricular arrhythmias.

Main Methods:

  • Noninvasive evaluation of 18 patients with malignant ventricular arrhythmias.
  • Comparison of encainide and flecainide efficacy using Holter monitoring and exercise criteria.

Main Results:

  • Both encainide and flecainide significantly reduced spontaneous ventricular arrhythmias.

  • Concordance in drug effect was 61% by Holter and 50% by exercise criteria.
  • 67% of patients responded to encainide, while 44% responded to flecainide; overall concordance was 56%.
