    Summary

    Thyrotropin Releasing Hormone (TRH) showed transient muscle tone increases and dyspnea in Amyotrophic Lateral Sclerosis (ALS) patients. No improvements in muscle strength or nerve reflexes were observed during the trial.

    Background:

    • Amyotrophic Lateral Sclerosis (ALS) is a progressive neurodegenerative disease affecting motor neurons.
    • Thyrotropin Releasing Hormone (TRH) is a hypothalamic hormone with potential roles beyond the pituitary axis.
    • Investigating novel therapeutic agents for ALS is crucial due to limited treatment options.

    Purpose of the Study:

    • To evaluate the safety and efficacy of Thyrotropin Releasing Hormone (TRH) in patients with Amyotrophic Lateral Sclerosis (ALS).
    • To assess the effects of TRH on muscle tone, reflexes, and voluntary strength in ALS patients.

    Main Methods:

    • A trial involving subcutaneous administration of TRH (5.0 mg/kg) every other day for two weeks.
    • Assessment of muscle tone, plantar reflexes, F-waves, H-reflexes, and muscle strength (MRC and strain gauge testing).
    • Monitoring for adverse events, including respiratory changes.

    • Transient increase in muscle tone observed in ALS patients following TRH injection.

    • Further investigation is needed to understand the safety profile and potential mechanisms of TRH side effects in neurological conditions.
