[Distal posterior cerebral artery aneurysm at the P4 segment: a case report]

Observation:

  • A 77-year-old male presented with sudden headache and computed tomography revealed complex intracranial hemorrhages (intracerebral, intraventricular, subarachnoid).
  • Initial magnetic resonance angiography was inconclusive; however, rebleeding led to conventional angiography, identifying a small 2-mm PCA aneurysm at the P4 segment.
  • The patient's renal dysfunction precluded conventional angiography initially.

Findings:

  • P4 segment PCA aneurysms are characterized by their small size and a distinct hematoma distribution pattern involving occipital intracerebral, intraventricular, and subarachnoid hemorrhage.
  • Endovascular parent artery occlusion using n-butyl-2-cyanoacrylate was successfully performed for the P4 segment aneurysm.

Implications:

  • The characteristic hematoma pattern associated with P4 segment aneurysms necessitates conventional angiography for definitive diagnosis.
  • Endovascular techniques, such as parent artery occlusion, offer a viable treatment option for select distal PCA aneurysms.
  • Early diagnosis and appropriate management are critical for patients presenting with symptoms suggestive of P4 segment PCA aneurysms.
