Pure laparoscopic liver resection for large malignant tumors: does size matter?

Laparoscopic liver resection (LLR) is safe and effective for large and giant malignant liver tumors. This minimally invasive approach offers acceptable outcomes with low morbidity and good oncologic results.

- Median operative time was 240 minutes, with a median blood loss of 500 ml. Eight conversions (15.4%) and six complications (11.5%) were reported. No 90-day mortality occurred.
- Giant tumors showed significantly greater blood loss (p=0.002) and a trend towards longer operative time (p=0.052) but similar rates of conversion and complications compared to large tumors.
Conclusions:
- Laparoscopic liver resection (LLR) is a feasible and safe approach for managing large malignant liver tumors.
- LLR demonstrates acceptable morbidity and oncologic outcomes for large malignant liver tumors.
- For giant malignant liver tumors, LLR is associated with increased blood loss and operative time but not increased complications.
