Serum-ionized calcium levels are lower in neonates with poor outcomes from hypoxic-ischemic encephalopathy (HIE) treated with brain hypothermia (BHT). Lower calcium levels during BHT correlate with worse prognosis, but BHT may offer protection.

Purpose of the Study:
- To investigate the relationship between serum-ionized calcium levels and neurological prognosis in HIE neonates undergoing BHT.
- To analyze changes in serum-ionized calcium before and during BHT.

- Serum samples were collected from 16 neonates with HIE treated with BHT.
- Developmental testing was performed at 18 months of age to assess outcomes.
- Serum-ionized calcium concentrations were measured before and during BHT.

- Neonates with poor outcomes (Developmental Quotient [DQ] <80) had significantly lower serum-ionized calcium levels than those with good outcomes (DQ >80).
- Serum-ionized calcium levels decreased significantly in both groups during 6 hours of BHT.
- A lowest adjusted serum-ionized calcium concentration below 1.05 mmol/L, a known poor outcome indicator, was observed in 57.1% of the good outcome group.
Conclusions:
- Calcium influx into cells continues during early BHT, irrespective of neurological prognosis.
- BHT may offer neuroprotection against neurological damage in high-risk neonates even with low serum-ionized calcium concentrations.
