Background:

  • Cutaneous leishmaniasis is spreading to non-endemic areas, increasing the need to understand its immune response.
  • Reduced CD1a receptor expression on Langerhans cells is observed in various skin infections.

Purpose of the Study:

  • To investigate the immune response in cutaneous leishmaniasis across different Ridley patterns and parasitic loads.
  • To analyze the expression of CD1a and other inflammatory markers in relation to disease severity.

Main Methods:

  • Skin biopsies from 33 patients with Leishmania tropica or L. major infection were analyzed.
  • Immunohistochemistry was used to assess CD1a, CD68, CD3, CD4, CD8, CD20, and CD138 expression.
  • CD1a was quantified, and other markers were graded based on inflammatory infiltrate.

Main Results:

  • CD1a expression decreased 14-fold with increasing parasitic indices and was significantly lower in the epidermis compared to normal skin.
  • Dermal CD1a Langerhans cells increased, and amastigotes showed strong CD1a/CD68 staining.
  • The primary inflammatory infiltrate comprised macrophages and double-negative CD3 T lymphocytes.

Conclusions:

  • A significant decrease in CD1a expression is linked to cutaneous leishmaniasis, potentially due to parasite uptake or regulatory T cell activity.
  • Modulating the immune microenvironment in cutaneous leishmaniasis offers potential therapeutic and prophylactic strategies.
