Summary

A rare case of fetal endometriosis, presenting as a large pelvic mass, was diagnosed antenatally. Surgical removal confirmed the diagnosis, highlighting the importance of considering endometriosis in fetal abdominal masses.

Area of Science:

  • Reproductive Medicine
  • Pediatric Surgery
  • Fetal Medicine

Background:

  • Fetal abdominal masses present a diagnostic challenge due to a broad differential.
  • Fetal endometriosis is exceptionally rare, with limited case reports globally.
  • Accurate antenatal diagnosis and counseling are crucial for managing these complex cases.

Observation:

  • A large fetal pelvic mass was diagnosed at 35 weeks gestation.
  • The mass demonstrated significant enlargement by 37 weeks, necessitating intervention.
  • Postnatal examination revealed a 7.0 × 4.5 cm cyst-like structure.

Findings:

  • Final pathology confirmed a hemorrhagic ovarian cyst with focal endometriosis.
  • Surgical intervention involved a left salpingo-oophorectomy on day of life 2.

  • This case represents one of the few documented instances of fetal endometriosis in the United States.
  • Implications:

    • This case underscores the importance of including endometriosis in the differential diagnosis of fetal pelvic masses.
    • Improved understanding of imaging characteristics can enhance antenatal diagnosis and patient counseling.
    • Further case reports are vital for refining diagnostic and management strategies for rare fetal abdominal conditions.
