Summary

Laparoscopic biliary bypass offers a safe and effective palliative option for distal biliary obstruction when surgery is not feasible. This minimally invasive approach provides symptom relief and improves quality of life with low complication rates.

Background:

Purpose of the Study:

  • To evaluate the efficacy and safety of laparoscopic biliary diversion for palliative treatment of distal biliary obstruction.
  • To assess perioperative outcomes and complications in patients unsuitable for pancreatoduodenectomy.

Main Methods:

  • A retrospective analysis of 12 patients undergoing laparoscopic biliary decompression for distal biliary obstruction between 2008 and 2014.
  • Patients were unsuitable for pancreatoduodenectomy.
  • Perioperative variables and 30-day outcomes were prospectively collected.

Main Results:

  • 12 patients with distal biliary obstruction underwent laparoscopic palliative bypass (10 hepatico-jejunal, 2 choledochoduodenostomies).
  • No conversions to open surgery were required.
  • Mean operative time was 85 minutes, blood loss 75 ml, and hospitalization 4.5 days, with low complication rates (Clavien Dindo Class II and IIIb).

Conclusions:

  • Laparoscopic biliary bypass is a viable option for managing distal biliary obstructions in selected patients.
  • It offers low perioperative morbidity, effective symptom palliation, and improved quality of life.
