    Background:

    • Laparoscopic plication of the gastric big curvature (LP GBC) is a novel restrictive bariatric procedure.
    • Its mechanism is similar to laparoscopic sleeve gastrectomy (LSG), but potentially avoids suture insufficiency complications.

    Purpose of the Study:

    • To compare the efficacy and safety of LP GBC versus LSG.
    • To evaluate complications, body mass index (BMI) loss, excess BMI loss, appetite reduction, and changes in comorbid conditions.

    Main Methods:

    • Comparative analysis of patients undergoing LP GBC and LSG.
    • Assessment of clinical outcomes including BMI changes, excess BMI loss, and comorbid disease severity at 2 and 3 years post-surgery.

    Main Results:

    • At 2 years, excess BMI loss was 42.4% for LP GBC and 78.9% for LSG.
    • At 3 years, excess BMI loss was 28.5% for LP GBC and 72.8% for LSG.
    • LP GBC demonstrated a lower efficacy in excess weight reduction compared to LSG.

    Conclusions:

    • Laparoscopic plication of the gastric big curvature is less effective than laparoscopic sleeve gastrectomy for morbid obesity treatment.
    • LSG appears to be a more effective bariatric procedure for significant and sustained weight loss.
