Summary

A new instrument, the Pressure Ulcer Prevention Inventory (PUPI), was developed to objectively identify avoidable and unavoidable hospital-acquired pressure ulcers (HAPUs). This tool aids in root-cause analysis and adverse event reporting for better patient care.

Background:

  • Hospital-acquired pressure ulcers (HAPUs) remain a significant challenge in acute care despite existing prevention strategies.
  • Differentiating avoidable from unavoidable HAPUs is crucial for accurate quality assessment and improvement.

Purpose of the Study:

  • To develop an operational definition for avoidable/unavoidable HAPUs.
  • To create and validate an instrument, the Pressure Ulcer Prevention Inventory (PUPI), for identifying HAPUs as avoidable or unavoidable in acute care settings.

Main Methods:

  • The PUPI instrument was developed and underwent psychometric testing.
  • A retrospective pilot study involving 31 hospitalized adult patients with HAPUs was conducted using the PUPI.
  • Content validity, criterion validity, construct validity, and interrater reliability of the PUPI were assessed.

Main Results:

  • The PUPI demonstrated excellent content validity (CVI=0.99) and acceptable criterion and construct validity.
  • Interrater reliability was high, with perfect agreement on total PUPI scores (κ=1.0) and 93% agreement on individual items.
  • No specific risk factors were significantly associated with unavoidable HAPUs in this study.

Conclusions:

  • An operational definition and a validated instrument (PUPI) for classifying HAPUs as avoidable or unavoidable were successfully developed.
  • The PUPI offers an objective, structured method for HAPU assessment.
  • This instrument can enhance root-cause analyses and reporting of adverse pressure ulcer events.
