Insights
A patient with a Björk-Shiley mitral valve prosthesis experienced systemic embolization due to a left atrial thrombus, despite optimal anticoagulation and antiplatelet therapy. This highlights potential risks even with standard medical management after valve replacement.
Area of Science:
- Cardiology
- Cardiovascular Surgery
- Biomedical Engineering
Background:
- Mitral valve replacement with prosthetic devices like the Björk-Shiley prosthesis is a common procedure.
- Optimal anticoagulation and antiplatelet therapy are standard post-operative management strategies to prevent thromboembolic events.
- Systemic embolization is a significant potential complication following prosthetic heart valve implantation.
Observation:
- A 20-year-old male presented with systemic embolization 11 months after mitral valve replacement.
- Echocardiography identified a left atrial thrombus adherent to the posterior wall.
- The prosthetic valve function was noted to be normal.
Findings:
- A significant left atrial thrombus was confirmed as the source of embolization.
- The thrombus formation occurred despite the patient being on optimal anticoagulation and antiplatelet therapy.
- Surgical confirmation validated the echocardiographic findings.
Implications:
- This case underscores the risk of thrombus formation and subsequent embolization in patients with mechanical mitral valve prostheses, even with appropriate medical management.
- Further research into risk factors and preventative strategies for left atrial thrombus in this population may be warranted.
- This finding emphasizes the importance of vigilant monitoring and potential surgical intervention in cases of prosthetic valve-related thromboembolism.
Abstract:
A 20-year-old male with a Björk-Shiley mitral valve prosthesis on optimal anticoagulation and antiplatelet drug therapy presented with systemic embolization 11 months after mitral valve replacement. Cross-sectional and Doppler echocardiography revealed a left atrial thrombus attached to the posterior wall of the left atrium and normal function of the prosthetic valve. The findings were confirmed at surgery.
