Intraocular pressure (IOP) is the primary risk factor for primary open angle glaucoma (POAG) progression. Age, hypertension, and female sex also significantly impact POAG development and worsening.

Background:

  • Primary open angle glaucoma (POAG) is a leading cause of irreversible blindness.
  • Understanding individual risk factors is crucial for predicting POAG progression and managing patient outcomes.

Purpose of the Study:

  • To identify and describe individual risk factors associated with POAG progression.
  • To evaluate the impact of these factors on visual field (Mean Deviation) and neural fiber layer (Neural Fiber Index) damage.

Main Methods:

  • Retrospective, observational study of 190 Caucasian patients (377 eyes) with POAG.
  • Analysis of risk factors (age, IOP, gender, hypertension, familiarity, depression) and their correlation with Mean Deviation (MD) and Neural Fiber Index (NFI).
  • Progression evaluated using a General Linear Model over a mean follow-up of 79 months.

Main Results:

  • Age, intraocular pressure (IOP), female gender, hypertension, and familiarity were significant risk factors for worsening Mean Deviation (MD).
  • IOP and depression were significant risk factors for worsening Neural Fiber Index (NFI).
  • IOP emerged as the most significant factor influencing both MD and NFI progression.

Conclusions:

  • Intraocular pressure (IOP) remains the paramount risk factor for glaucoma progression.
  • Age, familiarity, and female sex are significant contributors to POAG development and progression.
  • Arterial hypertension requires careful consideration in clinical follow-ups for POAG patients.
