Summary

Patients with chronic neck pain exhibit altered respiratory function, including reduced lung volumes and pressures. Management strategies targeting neck pain may improve respiratory health.

Area of Science:

  • Physiotherapy
  • Respiratory Medicine
  • Musculoskeletal Disorders

Background:

  • Chronic neck pain (CNP) is a prevalent condition affecting a significant portion of the adult population.
  • Limited understanding exists regarding the impact of CNP on respiratory function.
  • Investigating the relationship between cervical musculoskeletal health and pulmonary capacity is crucial for comprehensive patient care.

Purpose of the Study:

  • To systematically review and synthesize existing evidence on respiratory function changes in individuals with chronic neck pain.
  • To identify specific respiratory parameters affected by CNP and their correlation with musculoskeletal and psychological factors.
  • To evaluate the effectiveness of respiratory interventions in patients with CNP.

Main Methods:

  • A systematic literature search was conducted across major scientific databases (MEDLINE, PubMed, Scopus, etc.) up to December 2015.
  • Inclusion criteria focused on English-language observational studies examining cervical musculoskeletal and respiratory parameters in patients with CNP.
  • Studies were assessed for methodological quality, including study design, sampling, and statistical analysis.
  • Main Results:

    • Nine observational studies met the inclusion criteria.
    • Patients with CNP demonstrated significantly lower maximum inspiratory and expiratory pressures compared to asymptomatic individuals.
    • Reduced respiratory volumes, correlations between muscle strength/endurance, cervical range of motion, psychological state, and respiratory parameters were observed.
    • Lower Pco2 levels and a relationship between chest expansion and neck pain were noted.

    Conclusions:

    • Functional pulmonary impairments are associated with chronic neck pain.
    • Respiratory retraining interventions showed efficacy in improving certain musculoskeletal and respiratory impairments.
    • Further research investigating the impact of CNP management on respiratory function is warranted.
