- Brain imaging revealed a small hemorrhage (1.6 ml) localized to the left LGB.

- The patient had risk factors including hypertension, polycythemia, dyslipidemia, smoking, and alcohol use.
- Literature review identified eight previously reported cases of LGB hemorrhage causing various visual field defects.
- This case represents one of the rarest forms of cerebral hemorrhage, with localized LGB hemorrhage occurring in only 1 of 2,763 stroke registry patients.
