Area of Science:

  • Ophthalmology
  • Oncology
  • Neurology

Background:

  • Sporadic Burkitt lymphoma (BL) is an aggressive non-Hodgkin lymphoma.
  • Relapsed BL typically presents with systemic or nodal involvement.
  • Optic neuropathy can have diverse etiologies, including neoplastic infiltration.

Observation:

  • A patient with a history of sporadic BL presented with isolated infiltrative optic neuropathy.
  • The optic neuropathy was the sole clinical manifestation of disease recurrence.
  • Ophthalmologic examination revealed signs consistent with infiltrative optic nerve involvement.

Findings:

  • This case represents the first reported instance of isolated infiltrative optic neuropathy as the sole indicator of sporadic BL recurrence.
  • The findings suggest that neoplastic infiltration of the optic nerve can occur without other signs of systemic relapse.

  • Diagnostic workup confirmed Burkitt lymphoma infiltration of the optic nerve.
  • Implications:

    • Clinicians should consider the possibility of lymphoreticular malignancy relapse in patients presenting with acute visual symptoms suggestive of optic neuropathy.
    • Ophthalmologists and oncologists should be aware of this rare presentation of BL recurrence.
    • Early recognition and diagnosis are crucial for timely management of relapsed BL presenting with optic neuropathy.
