Summary

Laparoscopic radical nephrectomy (LRN) is safe for renal tumors >7 cm. However, LRN for tumors >10 cm requires longer operating times and is technically demanding, with worse recurrence-free survival.

Background:

  • Laparoscopic radical nephrectomy (LRN) is a standard treatment for renal tumors.
  • The limitations of LRN for large renal tumors remain unclear.

Purpose of the Study:

  • To evaluate the safety and efficacy of LRN for renal tumors exceeding 7 cm, including those larger than 10 cm.

Main Methods:

  • A retrospective review of 126 patients who underwent LRN for renal tumors between March 2001 and September 2014.
  • Patients were categorized by tumor size: ≤4.0 cm, 4.1-7.0 cm, 7.1-10.0 cm, and >10.0 cm.
  • Surgical and oncological outcomes were compared across tumor size groups.

Main Results:

  • Operating time for tumors 7.1-10.0 cm was similar to smaller tumors, but significantly longer for tumors >10 cm.
  • No significant difference in complication rates was observed across tumor size groups.
  • Recurrence-free survival was worse for tumors >10 cm compared to smaller tumors.

Conclusions:

  • LRN can be safely performed for large renal tumors (>7 cm).
  • LRN for tumors >10 cm is technically challenging, necessitates longer operative durations, and is associated with poorer oncological outcomes.
