Dermatofibrosarcoma protuberans is a rare, slow-growing breast tumor. Diagnosis requires histopathological and immunohistochemical analysis, with complete surgical excision for treatment.

- Dermatofibrosarcoma protuberans (DFSP) is a rare, locally aggressive skin tumor.

Observation:
- A 44-year-old female presented with a large (8x5.5 cm) breast mass.
- Mammography showed a well-defined, lobulated mass with skin involvement.
- Initial biopsy revealed a spindle cell lesion, prompting further investigation.
Findings:
- Macroscopic examination revealed an ulcerated mass with a well-defined, pale solid tumor.
- Microscopic analysis showed monotonous spindle cells in a storiform pattern, with CD34 positivity.
- Histopathology and immunohistochemistry confirmed the diagnosis of dermatofibrosarcoma protuberans.
Implications:
- Accurate diagnosis of DFSP is crucial for appropriate management.
- Complete surgical excision with wide margins is the standard treatment.
- High recurrence rates (20-50%) necessitate vigilant follow-up.
