The sense of smell is achieved through the activities of the olfactory system. It starts when an airborne odorant enters the nasal cavity and reaches olfactory epithelium (OE). The OE is protected by a thin layer of mucus, which also serves the purpose of dissolving more complex compounds into simpler chemical odorants. The size of the OE and the density of sensory neurons varies among species; in humans, the OE is only about 9-10 cm2.

Background:

Purpose of the Study:

  • To compare the Phenyl Ethyl Alcohol Smell Threshold Test (PEA) and the OLFACT-RL Odor Threshold Test.
  • To assess the correlation between these two unilateral olfactory tests in patients with chemosensory complaints.

Main Methods:

  • Retrospective chart review of 23 patients with chemosensory complaints.
  • Patients underwent olfactory testing using both PEA and OLFACT-RL tests.
  • Spearman correlation coefficients were used to analyze the data.

Main Results:

  • A significant correlation was found between PEA and OLFACT-RL values for the left nostril (r = 0.49, p = 0.0184).
  • Correlation was also observed in subgroups with hyposmia (excluding anosmia), without burning mouth syndrome, and without dysosmia.
  • No significant correlation was found between the tests for the right nostril.

Conclusions:

  • The Phenyl Ethyl Alcohol Smell Threshold Test and OLFACT-RL Odor Threshold Test may be interchangeable for left-sided olfactory threshold assessment.
  • Further research is needed to understand the lack of correlation in the right nostril.
