Dynamic and morphologic evaluation of erectile dysfunction on penile doppler sonography and contrast cavernosography

Vascular abnormalities are common causes of erectile dysfunction (ED) in Nigerian men. Penile Doppler sonography effectively identified arterial insufficiency and venous leakage in patients with ED.

Background:
- Erectile dysfunction (ED) is the persistent inability to achieve or maintain penile erection for sexual intercourse.
- It results from complex interactions between psychologic, hormonal, nervous, arterial, venous, and sinusoidal systems.
- Vascular factors are significant contributors to ED, particularly in certain geographic settings.
Purpose of the Study:
- To determine real-time morphologic vascular abnormalities in men with ED.
- To utilize penile Doppler sonography for assessing vasculogenic ED.
- To investigate the prevalence of specific vascular pathologies in Nigerian men with ED.
Main Methods:
- A review of 21 male patients referred for suspected vasculogenic ED.
- Examination of cavernorsal arteries using 7.5 MHz linear transducer in gray scale and duplex Doppler modes.
- Intracavernosal injection of 60 mg papaverine followed by serial peak systolic velocity (PSV), end diastolic velocity (EDV), and diameter measurements for 30 minutes.
Main Results:
- Out of 21 patients, 5 had normal findings.
- 10 patients exhibited evidence of venous leakage.
- 5 patients presented with arterial insufficiency, including 3 with tunica albuginea calcifications suggestive of Peyronie's disease. One patient had combined arterial and venous issues.
- Patients with arterial insufficiency were older and often had comorbidities like diabetes and hypertension.
Conclusions:
- Vascular etiologies significantly contribute to ED in the studied population.
- Papaverine-induced penile Doppler sonography is a promising tool for accurate assessment of ED.
- This diagnostic approach aids in the overall management of patients with vasculogenic ED.
