Hypertensive heart failure (HHF) commonly causes right ventricular (RV) systolic dysfunction, impacting over half of patients. This dysfunction worsens with reduced left ventricular ejection fraction (LVEF), highlighting RV assessment importance in HHF management.

Area of Science:

  • Cardiology
  • Echocardiography
  • Heart Failure Research

Background:

  • Hypertensive heart failure (HHF) is a leading cause of heart failure (HF) admissions in sub-Saharan Africa.
  • Right ventricular (RV) dysfunction is an independent predictor of adverse outcomes in HF patients.
  • Understanding RV systolic function in HHF is crucial for improving patient prognosis.

Purpose of the Study:

  • To determine the prevalence of RV systolic dysfunction in patients with HHF.
  • To assess RV systolic function using multiple echocardiographic parameters.
  • To investigate the relationship between RV systolic dysfunction and left ventricular ejection fraction (LVEF) in HHF.

Main Methods:

  • One hundred HHF patients and 50 controls underwent physical examination, blood tests, ECG, and transthoracic echocardiography.

Related Experiment Videos

  • RV systolic function was evaluated using tricuspid annular plane systolic excursion (TAPSE), RV myocardial performance index (MPI), and tissue Doppler systolic excursion (S').
  • Standard American Society of Echocardiography guidelines were followed for RV assessment.
  • Main Results:

    • RV systolic dysfunction was prevalent in 53% of HHF subjects by TAPSE, 56% by RV MPI, and 48% by RV S'.
    • A significant correlation was observed between RV systolic dysfunction and reduced LVEF in HHF patients.
    • These findings indicate widespread RV functional impairment in HHF.

    Conclusions:

    • A high proportion of HHF patients exhibit RV systolic functional abnormalities.
    • RV systolic dysfunction is linked to reduced LVEF in HHF.
    • Echocardiographic assessment of RV function is vital for managing HHF.
