Severe rosacea, often involving the eyes, can be mistaken for lupus erythematosus. Treatment with antibiotics and anti-inflammatory eye drops proved effective, with Demodex mites potentially contributing to severe cases.

Severe oculofacial rosacea can present with extensive facial and ocular eruptions.
Misdiagnosis as lupus erythematosus (LE) is possible due to similar rash presentations.

Observation:

A 28-year-old female with severe oculofacial rosacea initially treated for lupus erythematosus experienced lesion exacerbation.
Histopathology confirmed rosacea with Demodex folliculorum infection.

Findings:

Successful treatment involved oral doxycycline, steroid eye drops, and ocular lubricants.
A relapse four years later responded to oral tetracycline, steroid eye drops, and ocular lubricants.

Implications:

Rosacea can be severe and disfiguring, mimicking other conditions.
