Elderly patients are at risk of press-through package (PTP) tablet complications. A rare case shows PTP ingestion caused small bowel perforation and fistula formation, highlighting the need for careful PTP use.
Area of Science:
- Gastroenterology
- Surgical Case Reports
Background:
- Increased use of press-through package (PTP) tablets has led to more mis-swallowing incidents, particularly in the elderly.
- PTP ingestion can result in serious gastrointestinal complications.
Observation:
- A healthy 62-year-old male presented with persistent abdominal pain.
- Abdominal CT revealed a radiopaque foreign body in the small intestine.
- Laparoscopic surgery identified a PTP fragment causing perforation and fistula formation.
Findings:
- The foreign body was confirmed as a press-through package (PTP) tablet fragment.
- The PTP fragment caused significant edema, perforation, and adhesions in the distal small intestine.
- The patient was successfully treated with laparoscopic small bowel resection.
Implications:
- This case highlights a rare but severe complication of inadvertent PTP ingestion.
- Increased awareness and precautions are needed to prevent PTP-related injuries, especially in elderly patients.
- Safe handling and administration of PTP medications are crucial to avoid iatrogenic injury.

Clinical Manifestations:
Inflammatory Bowel Disease I: Ulcerative Colitis
Inflammatory bowel disease, or IBD, encompasses a group of disorders characterized by chronic inflammation or ulceration of the gastrointestinal tract.
Risk Factors
The exact cause of IBD remains unclear, although it is believed to be due to a mix of genetic, environmental, microbial, and immune factors. Genetic factors are significant in determining susceptibility to IBD, with family history being a critical risk factor. Individuals with a first-degree relative who has IBD are at...

Inflammatory Bowel Disease II: Crohn's Disease
Inflammatory bowel disease, commonly known as IBD, refers to a collection of disorders that lead to persistent inflammation of the gastrointestinal tract. The two types of IBD are ulcerative colitis, which impacts the colon, and Crohn's disease, which can involve any part of the gastrointestinal segment.
