[A Case of Adrenal Leiomyosarcoma]

Observation:

• A 61-year-old male presented with abdominal pain and a 7 cm left adrenal mass on enhanced CT and MRI.
• The mass exhibited heterogeneous enhancement and abnormal fluorodeoxyglucose (FDG) uptake on PET scan.
• No endocrinological abnormalities were detected preoperatively.

Findings:

• Preoperative diagnosis suggested left adrenocortical carcinoma (cT2N0M0).
• Pathological examination of the resected specimen revealed leiomyosarcoma of the adrenal gland.
• The patient remained disease-free with no local recurrence or metastasis at 16 months post-surgery.

Implications:

• This case highlights the importance of considering rare adrenal tumors beyond adrenocortical carcinoma.
• Accurate pathological diagnosis is essential for appropriate treatment and patient management.
• Successful surgical management of adrenal leiomyosarcoma can lead to favorable long-term outcomes.
