Improving virtual temporal bone drilling realism depends on the manipulandum, not just haptic device fidelity. Enhanced manipulandum design significantly improved user perception of realism and educational value in surgical simulations.
Area of Science:
- Neurosurgery
- Medical Simulation
- Haptics Technology
Background:
- Virtual temporal bone simulations often lack realistic drill-bone interaction.
- Haptic device and manipulandum fidelity influence perceived realism in virtual surgery.
- This study evaluates the impact of hardware upgrades on surgical simulation realism.
Purpose of the Study:
- To assess the effect of haptic device and manipulandum fidelity on the realism of virtual temporal bone drilling.
- To compare a 3-degrees of freedom (DOF) haptic device with a 6-DOF device using standard and modified manipulandums.
- To determine which hardware component contributes more to the perceived realism of the simulation.
Main Methods:
- Six otologic surgeons performed virtual mastoidectomies using different haptic hardware setups.
- A 6-DOF HD² haptic device was tested with both a standard and a modified otic drill manipulandum.
- Surgeons' perceptions of realism were collected and analyzed using statistical tests.
Main Results:
- The modified HD² manipulandum was significantly favored for acoustic properties and overall bone similarity.
- No significant preference was found for increased haptic device fidelity.
- Users preferred the modified manipulandum for osseous feel, vibration, overall realism, education, and rehearsal.
Conclusions:
- Enhancing manipulandum realism is crucial for improving virtual surgical simulation.
- Increased haptic device fidelity alone does not proportionally increase perceived realism.
- There may be a limit to the fidelity achievable in current virtual haptic drill-bone interactions.
