Background:

  • Insomnia is a common sleep disorder affecting a significant portion of the population.
  • Understanding the prevalence and associated factors of insomnia in primary care is essential for effective intervention.

Purpose of the Study:

  • To determine the prevalence of insomnia in primary care attendees in Kerala, India.
  • To identify clinical correlates associated with subclinical and clinical insomnia.

Main Methods:

  • A cross-sectional survey was conducted involving 7017 adult patients across 71 primary health centers.
  • Insomnia Severity Index, socio-demographic data, chronic medical illnesses, mental disorders, disability, and life satisfaction were assessed.

Main Results:

  • Subclinical and clinical insomnia were reported by 17.7% and 4.7% of subjects, respectively.
  • Insomnia was associated with older age, female gender, urban background, lower education, chronic medical and mental disorders, greater disability, and poor life satisfaction.
  • Depressive disorder was a significant correlate of both clinical and subclinical insomnia.

Conclusions:

  • Insomnia, in both its clinical and subclinical forms, is a common and significant public health concern in primary care.
  • The findings underscore the need for prioritized attention and management of insomnia due to its association with substantial morbidity.
