Electronic Cigarette Use and Progression From Experimentation to Established Smoking

Adolescent electronic cigarette (e-cigarette) users who have tried conventional cigarettes are more likely to become established smokers. E-cigarette use appears to encourage, not deter, progression to regular cigarette smoking in teens.

- Adjusted models showed e-cigarette use significantly predicted current established smoking (OR: 1.80; 95% CI: 1.04-3.12).
- Associations with established smoking (OR: 1.57; 95% CI: 0.99-2.49) and past 30-day smoking (OR: 1.32; 95% CI: 0.99-1.76) did not reach statistical significance.
Conclusions:
- Among adolescent cigarette experimenters, e-cigarette use is independently associated with progression to current established smoking.
- Findings suggest e-cigarettes may encourage, rather than divert from, cigarette smoking initiation and progression in this population.
- E-cigarette use represents a potential gateway to sustained conventional smoking among adolescents.
