    Background:

    • Graduate medical education financing lacks transparency and accountability.
    • Federal investments have not effectively aligned physician workforce production with national healthcare needs.
    • The Teaching Health Center Graduate Medical Education (THCGME) program offered a chance to improve financing mechanisms.

    Purpose of the Study:

    • To develop a standardized methodology for quantifying the investment required to train primary care physicians in underserved areas.
    • To create a transparent and accountable financing model for residency training.

    Main Methods:

    • Designed the THCGME Costing Instrument using site visits, financial data, and expert input.
    • Collected data on educational outlays, patient service costs, revenues, and payer mix.
    • Fielded the instrument in 43 THCGME-funded residency programs from April to November 2015.

    Main Results:

    • 84% of programs (36/43) submitted the THCGME Costing Instrument.
    • Collected standardized data on residency labor, administration, educational costs, ambulatory care, patient service expenses, and resident-generated revenue.
    • Instrument data provides a more comprehensive view of costs compared to traditional Medicare cost reports.

    Conclusions:

    • The THCGME Costing Instrument offers a model for calculating evidence-based costs and revenues in community-based residency programs.
    • Enhances accountability by estimating residency costs and revenues across diverse settings.
    • Demonstrates feasibility and utility for application in other residency training contexts.
