Baseline Thromboelastogram as a Predictor of Left Ventricular Assist Device Thrombosis

Thromboelastography (TEG) maximum amplitude (MA) hypercoagulability at left ventricular assist device (LVAD) implantation did not predict pump thrombosis in this study. This novel concept showed no significant association between baseline TEG-MA and LVAD thrombosis risk.

- A retrospective, nested case-control study included 111 participants (37 case-control sets) from 2005 to 2015.
- Cases with LVAD pump thrombosis were matched to two controls based on age, sex, and follow-up duration.
- Multivariable logistic regression analysis calculated the odds ratio (OR) and 95% confidence interval (CI) for the association between TEG-MA hypercoagulability and thrombosis.
Main Results:
- TEG-MA hypercoagulability was observed in 10.8% of the thrombosis case group and 6.8% of the control group.
- No significant association was found between TEG-MA hypercoagulability at LVAD implantation and the occurrence of LVAD pump thrombosis (OR 1.71, 95% CI 0.42-7.05, p = 0.46).
Conclusions:
- Baseline TEG-MA hypercoagulability does not appear to be a reliable predictor of LVAD pump thrombosis.
- Further research may be needed to identify effective biomarkers for predicting LVAD thrombosis risk.
