Positional Obstructive Sleep Apnea (POSA) prevalence is similar when studied at home using respiratory polygraphy (RP). POSA patients have lower BMI and milder sleep apnea, often not requiring CPAP therapy.

  • Obstructive Sleep Apnea (OSA) is a common sleep disorder.
  • Positional OSA (POSA) is a subtype influenced by sleeping position.
  • Home-based respiratory polygraphy (RP) offers a convenient diagnostic method.

Purpose of the Study:

  • To determine the prevalence of POSA using self-administered home-based RP.
  • To compare characteristics of POSA patients with non-POSA patients.

Main Methods:

  • Retrospective study of 200 RP records over 52 months.
  • Analysis of demographic data, Epworth Sleepiness Scale, apnea-hypopnea index, and oxygen desaturation.
  • Comparison of POSA and non-POSA groups based on BMI, age, and time spent with oxygen saturation <90%.

Main Results:

  • 76% of patients diagnosed with OSA, 54.6% with POSA.
  • POSA patients were younger with significantly lower BMI (30.3 vs. 35.3, p<0.0001).
  • POSA group had less time with oxygen saturation <90% (8.8 vs. 28.7, p=0.0038) and spent 43% of recording time supine.

Conclusions:

  • Home-based RP shows a POSA prevalence comparable to sleep labs.
  • POSA patients typically have lower BMI, milder OSA, and less oxygen desaturation.
  • POSA patients are less likely to be prescribed CPAP therapy.
