Chronic kidney disease (CKD) is linked to increased bone marrow adiposity (BMAT). This association, particularly in advanced CKD stages, may be partly explained by higher sclerostin levels. Further research is needed to understand these mechanisms.
Area of Science:
- Bone biology and metabolism
- Nephrology
- Gerontology
Background:
- Bone marrow adiposity (BMAT) is associated with aging, osteoporosis, and reduced hematopoiesis.
- Mechanisms influencing BMAT, especially in chronic kidney disease (CKD), are not well understood.
- CKD may impact BMAT through lean mass loss or elevated sclerostin.
Purpose of the Study:
- To investigate the association between kidney function (eGFR) and vertebral BMAT in older adults.
- To explore the potential role of sclerostin in mediating the relationship between CKD and BMAT.
Main Methods:
- Cross-sectional study of 475 older adults from the AGES-Reykjavik cohort.
- 1H-MRS was used to measure vertebral BMAT (L1-L4).
- Linear regression models analyzed the association between eGFR and BMAT, adjusting for covariates and sclerostin in a subgroup.
Main Results:
- Higher BMAT was observed in individuals with eGFR <45 mL/min/1.734 cm2 compared to those with eGFR >60.
- No significant difference in BMAT was found between eGFR 45-60 and eGFR >60 groups.
- Adjusting for sclerostin attenuated the association between low eGFR (<45) and higher BMAT.
Conclusions:
- CKD stage 3b or worse is associated with increased bone marrow adiposity.
- Sclerostin may partially mediate the relationship between advanced CKD and greater BMAT.
- These findings highlight a potential link between kidney function decline and bone marrow fat accumulation.
