Summary

Non-typable Haemophilus influenzae can cause urinary tract infections and septicemia, even in adults with kidney stones. Prompt antibiotic treatment is crucial, as this pathogen may be increasingly resistant.

Area of Science:

  • Medical Microbiology
  • Infectious Diseases
  • Urology

Background:

  • Haemophilus influenzae typically causes respiratory infections.
  • Non-typable strains are now a leading cause of invasive disease post-Hib vaccine.
  • Rarely reported as a cause of urinary tract infections (UTIs).

Observation:

  • A case of an adult male with non-typable Haemophilus influenzae septicemia and UTI.
  • Patient presented with flank pain, CT revealed bilateral renal and right ureteral stones causing obstruction.
  • Haemophilus influenzae identified in blood and urine cultures.

Findings:

  • The isolated strain, despite rising resistance trends, remained susceptible to all tested antibiotics.
  • Successful treatment involved intravenous cefuroxime, ureteric stenting, and oral ciprofloxacin.

  • The patient recovered fully from the infection and obstruction.
  • Implications:

    • Highlights Haemophilus influenzae as a potential urinary pathogen, particularly with urinary tract abnormalities.
    • Emphasizes the need for awareness of antibiotic resistance patterns in Haemophilus influenzae.
    • Suggests considering Haemophilus influenzae in the differential diagnosis of UTIs, especially in at-risk patients.
