This case highlights pediatric Sweet Syndrome, a rare skin condition presenting with fever and painful plaques. Early recognition and treatment with corticosteroids led to rapid improvement in a young patient.
Area of Science:
- Dermatology
- Pediatrics
- Rheumatology
Background:
- Sweet's syndrome is a rare pediatric dermatosis of unknown cause.
- It is characterized by fever, neutrophilia, and painful skin plaques.
- Histologically, it involves dermal infiltration of neutrophils.
Observation:
- A 3-year-old female with complex chromosomopathy presented with fever and bullous skin lesions.
- Initial treatment for suspected impetigo was ineffective.
- Physical exam revealed erythematous-violaceous plaques and nodules with bullae.
Findings:
- Laboratory tests showed leukocytosis with marked neutrophilia and elevated CRP.
- Skin biopsy confirmed histopathological findings consistent with Sweet's Syndrome.
- Treatment with prednisone resulted in a significant clinical response.
Implications:
- Pediatric Sweet Syndrome requires a high index of suspicion when fever accompanies persistent skin lesions.
- While often idiopathic, associated conditions like proliferative disorders, infections, and immunodeficiencies should be screened for.
- Prompt diagnosis and corticosteroid treatment are crucial for managing pediatric Sweet Syndrome.
