Radiological Investigation III: Pulmonary Angiogram and PET Scan01:13

Radiological Investigation III: Pulmonary Angiogram and PET Scan

466
Radiological investigations are paramount in the diagnosis and management of various pulmonary diseases. Two essential investigations are the Pulmonary Angiogram and the Positron Emission Tomography (PET) Scan.
Pulmonary Angiogram
A Pulmonary Angiogram is an invasive procedure involving injecting a contrast medium through a catheter threaded into the pulmonary artery or the right side of the heart to visualize the pulmonary vasculature. Computed Tomography (CT) scans have mainly replaced this...

False Memories01:18

False Memories

465
False memories represent a cognitive distortion in which individuals recall events that did not happen, or remember them in an altered form. This phenomenon highlights the brain's constructive nature in processing and recalling memories, emphasizing that memory is not a perfect representation of past events but rather a dynamic reconstruction influenced by various factors.
One primary source of false memories is misattribution, where individuals incorrectly associate external information...

Leaky Scanning02:28

Leaky Scanning

5.7K
During most eukaryotic translation processes, the small 40S ribosome subunit scans an mRNA from its 5' end until it encounters the first start AUG codon. The large 60S ribosomal subunit then joins the smaller one to initiate protein synthesis. The location of the translation initiation is largely determined by the nucleotides near the start codon as there may be multiple translation initiation sites present on the mRNA.  Marilyn Kozak discovered that the sequence RCCAUGG (where R...

Muscles of the Vertebral Column01:27

Muscles of the Vertebral Column

3.2K
The back muscles that lie deep into the thoracolumbar fascia are called intrinsic or true back muscles. These muscles are divided into four layers: superficial, intermediate, deep, and deepest layers.
Superficial Layer:
The superficial layer consists primarily of the splenius muscles, which include the splenius capitis and splenius cervicis. These muscles are mainly responsible for the head and cervical spine movements, including extension, rotation, and lateral bending. The splenius capitis...

Articulations of the Vertebral Column01:28

Articulations of the Vertebral Column

3.1K
In addition to being held together by the intervertebral discs, adjacent vertebrae also articulate with each other at synovial joints formed between the superior and inferior articular processes called zygapophysial joints (facet joints). These are plane joints that provide for only limited motions between the vertebrae. The orientation of the articular processes at these joints varies in different regions of the vertebral column and serves to determine the types of motions available in each...

Vertebral Column: Regions and Curvature

6.0K
The vertebral column or spine is a flexible column that supports the head, neck, and body and  allows for their movements. It also protects the spinal cord.
Regions of the Vertebral Column
In an adult, the spine is subdivided into five regions: the cervical, the thoracic, the lumbar, the sacral, and the coccygeal region. The spine initially develops as a series of 33 vertebrae; after 20 years of age, the nine bones in the sacral region, five sacral, and four coccygeal bones fuse to form...

Summary

False positive FDG PET CT scans for skeletal metastasis can occur due to benign conditions like skeletal hemangioma. Combining bone scan and CT scan helps accurately diagnose these lesions, avoiding misdiagnosis of cancer.

Area of Science:

  • Nuclear Medicine
  • Radiology
  • Oncology

Background:

  • FDG PET CT scans are sensitive for detecting skeletal metastases but can yield false positives.
  • Benign conditions like skeletal hemangioma can mimic malignant lesions on FDG PET CT.
  • Skeletal hemangiomas are typically ametabolic, but rare hypermetabolic presentations exist.

Observation:

  • A case of skeletal hemangioma presenting with atypical hypermetabolic activity on FDG PET CT.
  • Standard imaging features of skeletal hemangioma on CT and MRI were present.
  • Bone scan showed no abnormal uptake, contrasting with the FDG PET CT findings.

Findings:

  • The hypermetabolic lesion on FDG PET CT was confirmed as a benign skeletal hemangioma.
  • CT and MRI demonstrated typical features of hemangioma, aiding in differential diagnosis.
  • Judicious use of complementary imaging modalities is crucial for accurate diagnosis.

Implications:

  • Highlights the potential for false positives in FDG PET CT for skeletal lesions.
  • Emphasizes the importance of integrating bone scan and CT findings to exclude malignancy.
  • Advocates for a multimodal imaging approach to improve diagnostic accuracy in skeletal imaging.
