Summary

Giant inguinoscrotal hernias (GIH) can now be treated with a novel minimally invasive approach. This combined technique of progressive pneumoperitoneum (PPP), botulinum toxin A injection (BTI), and enhanced view-totally extraperitoneal (eTEP) repair is safe and effective.

Background:

  • Giant inguinoscrotal hernias (GIH) significantly impair daily activities.
  • Previous repair methods for GIH lack minimally invasive options.
  • A novel multidisciplinary approach is explored for GIH repair.

Observation:

  • Two patients with severe GIH and loss of abdominal domain were treated.
  • Botulinum toxin A injection (BTI) was used for muscle paralysis, followed by progressive pneumoperitoneum (PPP) to increase abdominal domain.
  • Enhanced view-totally extraperitoneal (eTEP) access with transversus abdominis release facilitated dissection and mesh placement.

Findings:

  • The combined PPP, BTI, and eTEP approach allowed for successful retromuscular dissection and mesh repair.
  • Patients underwent reconstruction including partial scrotectomy and scrotoplasty.
  • Both patients tolerated the procedure well with no recurrence in the first postoperative year.

Implications:

  • This case series presents the first use of combined PPP, BTI, and eTEP for GIH repair.
  • The multidisciplinary approach demonstrates safety and efficacy in managing complex GIH.
  • This technique offers a promising minimally invasive solution for patients with GIH.
