Background:

Thyroid stimulating hormone (TSH)-suppressive therapy with levothyroxine (L-T4) is standard for thyroid cancer follow-up.
Balancing TSH suppression efficacy with L-T4 side effects is crucial.
Comorbidities can complicate TSH-suppressive therapy management.

Purpose of the Study:

To present a case study of managing TSH-suppressive therapy in a patient with papillary thyroid carcinoma and significant comorbidities.
To illustrate the challenges posed by cardiac conditions and altered drug absorption on TSH control.

Main Methods:

Case report of a 64-year-old male patient with papillary thyroid carcinoma.
Monitoring of TSH levels and cardiac status during levothyroxine therapy.
Consideration of coeliac disease's impact on intestinal absorption of L-T4.

Main Results:

The patient's cardiac condition worsened during TSH-suppressive therapy.
TSH concentrations fluctuated significantly due to altered intestinal absorption linked to coeliac disease.
