Extreme oncoplastic breast conserving surgery (EOBCS) enables breast conservation for patients with extensive disease, traditionally requiring mastectomy. While re-excision rates were notable, most patients achieved successful breast conservation with good cosmetic outcomes.
Area of Science:
- Oncology
- Surgical Oncology
- Breast Surgery
Background:
- Mastectomy is traditionally indicated for multiple breast lesions or tumors ≥5 cm.
- Patient preference for breast conservation drives interest in advanced surgical techniques.
- Perioperative outcomes of extreme oncoplastic breast conserving surgery (EOBCS) are not well-described.
Purpose of the Study:
- To evaluate the perioperative outcomes of EOBCS in patients with extensive breast cancer.
- To assess the feasibility of breast conservation in cases traditionally requiring mastectomy.
Main Methods:
- Observational cohort study of 111 breast cancer patients undergoing EOBCS (2012-2017).
- Evaluation of patient demographics, disease characteristics (multifocal, multicentric, tumor size), margin status, and re-operation rates.
- Assessment of cosmetic outcomes and oncologic recurrence rates.
Main Results:
- 82 patients had multifocal/multicentric disease (avg. 3.2 lesions), 18 had large unifocal tumors (>5 cm).
- Negative margins achieved in 78.3%; 51.4% required re-excision for inadequate margins.
- 12.6% underwent mastectomy, 37.8% had re-excision; 95% of conserved breasts had good/excellent cosmetic results.
- 1.1% recurrence rate in patients completing EOBCS and adjuvant radiation.
Conclusions:
- EOBCS offers a viable option for breast conservation in patients with extensive disease.
- Significant re-excision rates exist, but successful conservation with adequate margins is achievable for most.
- Further research is needed to confirm long-term oncologic safety and efficacy.
