Surgical clipping successfully treated a wide-neck basilar apex aneurysm and an associated anterior communicating artery aneurysm in a hypertensive smoker. Complete exclusion was confirmed at 1-year follow-up, with full recovery from temporary oculomotor nerve palsy.
