Summary

Serum levels of Fibroblast Growth Factor 21 (FGF21) were significantly higher in women with gestational diabetes mellitus (GDM). This finding suggests FGF21 may play a role in GDM pathophysiology, independent of insulin resistance.

Background:

  • Gestational diabetes mellitus (GDM) is a frequent endocrine complication during pregnancy.
  • Fibroblast growth factor 21 (FGF21), a metabolic hormone regulating glucose metabolism, is elevated in diabetes and insulin resistance.
  • The relationship between FGF21 and GDM requires further elucidation.

Purpose of the Study:

  • To compare serum FGF21 levels in women with and without GDM.
  • To investigate the correlation between serum FGF21 and insulin resistance in pregnant women.
  • To assess the predictive value of FGF21 for GDM.

Main Methods:

  • A cohort of 30 GDM patients and 60 healthy pregnant controls were analyzed.
  • Exclusion criteria included prior GDM history, hypertension, PCOS, organ failure, and relevant drug use.
  • Serum FGF21 levels were measured and correlated with glucose metabolism and insulin resistance parameters.

Main Results:

  • Serum FGF21 was significantly elevated in the GDM group (264.5±196.2 ng/L) compared to controls (59.1±36.5 ng/L).
  • No significant correlation was found between FGF21 levels and insulin resistance.
  • A FGF21 cut-off of 82.07 ng/L demonstrated 100% sensitivity and 85% specificity for GDM prediction.

Conclusions:

  • Serum FGF21 levels are increased in women with GDM, independent of insulin resistance.
  • FGF21 resistance may be directly implicated in the pathophysiology of GDM.
  • FGF21 shows potential as a biomarker for GDM prediction.
