This study reveals that bilateral chest trauma shares similar presentations, management strategies, and outcomes with unilateral chest trauma. Key predictors of mortality in blunt trauma include multiple rib fractures and intensive care unit admission.
Area of Science:
- Trauma Surgery
- Thoracic Surgery
- Emergency Medicine
Background:
- Bilateral chest trauma lacks specific literature compared to unilateral cases.
- This study aims to define the unique aspects, presentations, and outcomes of bilateral thoracic trauma.
- Investigating therapeutic algorithms for bilateral chest trauma is crucial.
Observation:
- A prospective, single-center study included 107 patients with bilateral chest trauma.
- Road traffic accidents were the most common cause (63.6%).
- Bilateral blunt trauma occurred in 67.3% and penetrating trauma in 32.7%.
Findings:
- Overall 30-day mortality was 14.9%.
- In blunt trauma, predictors of mortality included three or more rib fractures, pulmonary contusion, intubation, and ICU admission.
- Bilateral thoracic trauma demonstrated comparable presentation, investigation, management, and outcomes to unilateral cases.
Implications:
- Findings suggest that current trauma protocols adequately address bilateral chest trauma.
- Understanding predictors of mortality can guide resource allocation and treatment intensity.
- Further research may refine management strategies for specific bilateral thoracic injury patterns.
