This study highlights a successful trifocal intraocular lens (IOL) implant in a patient with a prior history of small incision lenticule extraction (SMILE) for high myopia. Novel IOL power calculation methods improved visual outcomes in this complex case.
Area of Science:
- Ophthalmology
- Refractive Surgery
- Intraocular Lens Technology
Background:
- Small incision lenticule extraction (SMILE) is a popular refractive surgery for high myopia.
- Cataract development post-refractive surgery presents unique challenges for intraocular lens (IOL) power calculation.
- Achieving spectacle independence after prior refractive surgery is a key patient goal.
Observation:
- A 54-year-old patient with a history of bilateral SMILE for high myopia developed cataracts.
- Post-SMILE aberrations were minimal, making the patient a candidate for trifocal IOL implantation.
- Standard IOL calculation methods were compared against a novel telecentric keratometry and swept-source OCT-based approach.
Findings:
- The IOL power predicted by the novel combined telecentric keratometry and swept-source OCT method was selected for implantation.
- One month post-operatively, the patient achieved excellent uncorrected distance visual acuity (UDVA) of 20/20p and near vision of N.6.
- The results suggest the superiority of newer IOL formulas in achieving satisfactory outcomes for post-refractive surgery patients.
Implications:
- Advanced IOL calculation formulas can optimize visual outcomes in patients with prior refractive surgery.
- Trifocal IOLs are a viable option for spectacle independence in carefully selected post-SMILE patients.
- This case underscores the importance of tailored refractive correction strategies in complex ophthalmic cases.

Introduction to Documentation and Reporting
Nursing documentation records essential information and details regarding a patient's care and treatment in written or electronic form. It is a critical aspect of nursing practice that involves documenting assessments, interventions, outcomes, and other relevant details about a patient's health status.
Documentation maps the patient's health journey by creating a comprehensive...
