Primary calvarial tuberculosis is a rare skull condition, often misdiagnosed. This case highlights a 26-year-old male with successful diagnosis and treatment of this rare osteomyelitis.

- Surgical excision and histopathological examination revealed tubercular pathology of the calvaria.

- Acid-fast bacilli in pus and biopsy material are crucial for diagnosis.
