Primary squamous cell carcinoma of the thyroid (PSCCT) is a rare and aggressive head and neck cancer. Early diagnosis and prompt treatment are crucial for improving outcomes in this deadly disease.

- Primary squamous cell carcinoma of the thyroid (PSCCT) is an exceptionally rare malignancy.
- Fewer than 60 cases of PSCCT have been documented in medical literature.
Observation:
- A 66-year-old female presented with compressive symptoms and shortness of breath.
- Physical exam revealed thyromegaly and vocal fold immobility.
- Imaging showed an enlarged thyroid, cervical lymphadenopathy, subglottic stenosis, and pulmonary nodules.
Findings:
- Histopathology and immunohistochemistry confirmed PSCCT.
- The patient had metastatic disease to the lung and cervical spine.
- The patient died shortly after palliative chemotherapy initiation.
Implications:
- PSCCT is an aggressive malignancy with a poor prognosis.
- Accurate diagnosis requires histopathologic and immunohistochemical evaluation.
- Early diagnosis and rapid treatment are essential for potentially improving survival in PSCCT patients.
