A woman experienced sudden left eyelid drooping (ptosis). CT scans revealed levator muscle inflammation, confirming isolated levator myositis. Steroid treatment successfully resolved the ptosis.

Area of Science:

  • Ophthalmology
  • Neurology
  • Medical Imaging

Background:

  • Ptosis, or eyelid drooping, can result from various neuromuscular and myogenic disorders.
  • Isolated levator myositis is a rare condition affecting the eyelid muscle.

Observation:

  • A 54-year-old female presented with acute onset left upper eyelid ptosis.
  • Clinical examination showed moderate ptosis, poor levator muscle function, and characteristic lid lag.
  • Ocular motility was unaffected.

Findings:

  • Computed tomography (CT) scan with contrast revealed enhancement of the left levator muscle and aponeurosis.
  • These findings, combined with clinical presentation, are characteristic of isolated levator myositis.
  • The patient's condition resolved completely within two weeks of systemic steroid treatment.

Related Experiment Videos

Implications:

  • This case highlights the diagnostic utility of CT imaging in identifying levator myositis.
  • Systemic corticosteroids are an effective treatment for isolated levator myositis.
  • Prompt diagnosis and treatment can lead to full recovery of eyelid function.
